From Chapter 4, 'Abbreviations'

4.3   In Footnotes and Endnotes

If possible, do not begin a note with an abbreviation which is normally printed in lower-case characters (‘e.g.’, ‘i.e.’, ‘pp.’). If this cannot be avoided, note that ‘c.’, ‘e.g.’, ‘i.e.’, ‘l.’, ‘ll.’, ‘p.’, ‘pp.’, remain entirely in lower case:

21 pp. 127–39. not 21 Pp. 127–39.

Other abbreviations, such as ‘Cf.’ or ‘Ibid.’, take an initial capital at the start of a note.
